Prior to participating in tryouts, students must turn in the following forms at least one week prior to tryouts:

• Current Sports Physical (within 12 months)

• Signed concussion form (one per school year)

• Proof of insurance form

Insurance and concussion forms from the prior school year are not valid. New forms must be completed at the start of each school year. All completed forms can be turned in to the office at Shepherd. During the month of July, all forms will need to be turned in at the District office on Main Street. All forms must be turned in at least one week prior to the tryout date. Students will not be allowed to try out for a team without the required paperwork.

Admission Prices

Shepherd Middle School charges admission to all home sporting events. Admission prices are listed below. Prices are subject to change based on the type of event (ie. tournaments). 

Adults- $3.00
Senior Citizens - $2.00
Students (K-8) - $1.00 

Sports passes are available for purchase at a discounted rate. Please talk to your child's coach for more information.
