Dear Parents:
Lincoln Elementary would love to hear from YOU!
You have been invited to complete the Illinois 5Essentials Parent Survey. The information you provide will help guide improvement as we strive to inspire each student to realize their potential through the education and opportunities we provide at Lincoln Elementary School.
Your participation in this survey is voluntary but crucial. We can’t receive results from the survey unless at least 20 percent of parents respond. The more responses parents provide, the more we will know how to focus our efforts toward the improvements you want. Therefore, we have set a goal to reach an 80 percent response rate. Please help us reach our goal by completing the survey.
• Go to: http://survey.5-essentials.org/illinois/survey/parent/ to take the survey.
• You may enter your email address if you would like to receive confirmation of your submission, but it is not required.
• Then enter “LaSalle” into the county field and “Lincoln Elem School-Ottawa” to find our school.
You will then be asked a series of questions to provide input regarding your family’s experience here at Lincoln Elementary School. Please remember to click “submit” once you are done so your responses can be counted. When a parent completes the 5Essentials Survey, please let your child's teacher know via School Status that you have completed it and your child will recieve $10 in Lincoln Loot!
Please know that your identity and responses will be kept confidential and will not be connected with your child. If you have multiple children who attend Lincoln, you may complete one survey for each child. However, only one parent per household may submit a survey for a given child. The survey is open now and closes on March 28, 2025, so please submit your responses soon!
Thank you in advance for your time and input. We appreciate you!

Dear Parents and Families,
Beginning February 18, teachers, parents and students across Illinois have an opportunity to participate in the Illinois 5Essentials Survey: Organizing Schools for Improvement. This survey was designed to generate a detailed picture of the inner workings of your child’s school. As a parent, this opportunity will allow you to share your thoughts on the important elements of school effectiveness in a survey about your child’s school.
The Illinois 5Essentials Survey will be administered online by UChicago Impact at the University of Chicago, on behalf of the Illinois School Board of Education. This survey identifies five indicators that can lead to important student outcomes, including improved attendance and larger test score gains. The five indicators that effect school success are:
• Effective Leaders
• Collaborative Teachers
• Involved Families
• Supportive Environments
• Ambitious Instruction
Research has shown that schools strong on these indicators, the `` 5Essentials” are 10 times more likely to improve student learning. School-level reports will be generated if at least 20 percent of parents respond to the survey and will be sent to schools and districts in the June and will be included in each school report card, released by ISBE. 5Essentials Reports for your school will also then be publicly available via the UChicago Impact website. 5Essentials Reports will display summary information, such as the percent of parents agreeing that they feel comfortable raising concerns to their child’s teacher. Parent and student identities will be kept completely confidential and responses will not be linked to any individual.
Your participation in the Illinois 5Essentials Survey will help us understand the conditions at your child’s school and guide improvement. Your identity and survey responses will be kept completely confidential and will never be connected to you or your child.
The Illinois 5Essentials Survey will be conducted February 18 through March 28, 2025.
For more information about the Illinois 5Essentials Survey, or to view sample school reports, please visit www.illinois.5-essentials.org. If you have any questions or concerns, please contact 5Essentials Client Services at 1-866-440-1874 or 5essentials@uchicago.edu.
Here are Lincoln, we feel that a team approach is best in the education of children. We value your input and, as always, use the information gatherer to make future improvements. Please help us by filling out the 5 Essentials survey using the information below:
